Description
Join us for a robust discussion about collaborative patient care. We will look into the role of the community-based pharmacist and how critical they are in collaborating with other healthcare professionals to support optimal care and improved patient outcomes. Pharmacists often experience a different view of the patient and their social determinants of health. We’ll hear evidence of how this important piece can contribute greatly to the whole healthcare team.

Objectives

At the completion of this activity, participants will be able to:

  • Define the scope of practice for pharmacists in South Carolina
  • Summarize the evidence supporting the role of pharmacists in collaborative patient care
  • Describe how prescribers can collaborate with community-based pharmacists to enhance patient outcomes
