Description
Hypertension is a long-recognized health burden in the US, and particularly the southeastern region of the US. Recent treatment guidelines have been developed to categorize blood pressure levels and treatment. This program addresses the implementation of the guidelines in primary care settings with a special focus for clinics serving underserved patient populations. Accreditation dates: 12/1/2023-11/30/2026
Objectives

At the completion of this activity, participants will be able to:

  • Explain the clinical value of out-of clinic blood pressure measurement in the management of hypertension
  • Describe the assessment of out-of-clinic blood pressure measurement in the primary care setting
  • Discuss the essential components in the incorporation of out-of-clinic blood pressure measurement in the primary care clinic
  • Recognize the importance of employing CPT codes in the management of hypertension
  • Describe the approach to diagnose elevated blood pressure for maximum patient care and hypertension control
  • Explain the incorporation of CPT codes and strategies in the efficient management of hypertension using SMBP
  • Recognize the importance of employing Self-Management of Blood Pressure (SMBP) in the clinic care setting
  • Describe the approach to diagnosis elevated blood pressure with SMBP
  • Identify the mechanism for the efficient management of hypertension using SMBP
  • State the importance of addressing undiagnosed hypertension in the clinic
  • Explain the approach to diagnosis elevated blood pressure in the primary care clinic
  • Identify the mechanism for the efficient management of hypertension and risk factors in the primary care setting
  • Discuss the impact of hypertension management on brain health
  • Describe the improved brain health with lower blood pressure levels
  • Examine the efficient management of hypertension and risk factors for improved brain health and brain function
