Description
As a result of heavy caseloads, time constraints, and fast-paced work environments, documentation can suffer, potentially affecting continuity of care, communication, accuracy and timeliness of communication. These challenges can contribute to minimal and unorganized documentation resulting in ineffective communication and risk of errors, misunderstandings which can ultimately impact the safety and care quality of those being served. Establishing clear documentation standards and regular training on "best practices" fosters and supports a culture that prioritizes thoroughness and quality, which in turn can make a significant difference in care management.
Objectives

At the completion of this activity, participants will be able to:

  • Identify the "best practice" components necessary to include in mental health and healthcare professionals' documentation.
  • Express the significance of accurate and timely documentation as a "best practices" in patient care.
  • Demonstrate how to document relevant information to effectively communicate a plan of care.
