Description
Join us to learn about the role pharmacists play in interprofessional teams and how they collaborate with others to improve patient outcomes. This session will discuss Federally Qualified Health Centers, 340B pharmacy programs, and the responsibilities of pharmacists in outpatient care. Through patient cases, participants will explore team-based care, state legislation, and practical interventions for outpatient settings in South Carolina.

Objectives

At the completion of this activity, participants will be able to:

  • Describe how Federally Qualified Health Centers differ from other outpatient medical offices
  • Recognize key difference between a 340B pharmacy and traditional retail pharmacy
  • Identify examples of pharmacist roles in an outpatient interdisciplinary team through real-life examples
  • Compare the role of pharmacists to other health professionals in the outpatient setting
  • Explore the team roles of ambulatory care and retail pharmacists through the lens of state legislation in South Carolina
  • Recommend interventions for care teams in different outpatient settings through patient cases
