Description
Hypertension is a long-recognized health burden in the US, and particularly the southeastern region of the US. Recent treatment guidelines have been developed to categorize blood pressure levels and treatment. This program addresses the implementation of the guidelines in primary care settings with a special focus for clinics serving underserved patient populations. Accreditation dates: 12/1/23-11/30/26
Objectives

At the completion of this activity, participants will be able to:

  • Recognize the importance of using a ‘hypertension team’ in the management and control of hypertension
  • Discuss the process for identification of the hypertension team members according to guidelines
  • Describe considerations for implementing the ‘hypertension team’ in the primary care setting
  • Recognize the importance of using a ‘hypertension team’ in the monitoring of hypertension management and control as recommended by treatment and management guidelines
  • Explain the mechanism and process for the implementation of an interactive hypertension team according to guidelines
  • Describe the components of the ‘hypertension team’ specific to the primary care setting
  • Recognize the importance of using a ‘hypertension team-based care’ for time management in hypertension management and control
  • Describe the synergistic value of the hypertension team in blood pressure management as a process improvement strategy
  • Identify the use of the ‘hypertension team’ for effective communication with the patients
