Description
Hypertension is a long-recognized health burden in the US, and particularly the southeastern region of the US. Recent treatment guidelines have been developed to categorize blood pressure levels and treatment. This program addresses the implementation of the guidelines in primary care settings with a special focus for clinics serving underserved patient populations. This program focuses on implementing guidelines in a primary care setting and incorporating Community Health Workers in the team. Objectives

At the completion of this activity, participants will be able to:

  • Analyze the implication of guidelines for primary care
  • Demonstrate the process to implement guidelines in the primary care clinic
  • Discuss implementing practice guidelines in the primary care setting
  • Determine the implications of accurate blood pressure measurement for primary care
  • Discuss implementing practice guidelines in the primary care setting with limited resources serving high risk patients
  • Illustrate implanting accurate blood pressure measurement protocols for primary care
  • Describe implementing the guidelines in clinical practice
  • Explain the roles of Community Health Workers and hypertension and diabetes control
  • Explain incorporating the Community Health Worker on the hypertension team
