Description
Hypertension is a long-recognized health burden in the US, and particularly the southeastern region of the US. Recent treatment guidelines have been developed to categorize blood pressure levels and treatment. This program addresses the implementation of the guidelines in primary care settings with a special focus for clinics serving underserved patient populations. This program focuses on policy development and implementation. Accreditation dates: 12/1/2023-11/30/2026
Objectives

At the completion of this activity, participants will be able to:

  • Describe the impact of Private Practice Physicians (PPPs) in the management of high blood pressure.
  • Identify resources needed to enable PPPs to be effective in managing hypertension
  • Discuss assessment of activities performed by the hypertension team in the private practice clinic.
  • Describe the importance of clinic workflow documentation regarding hypertension management
  • Identify workflow documentation of activities and personnel interactions
  • Assess impact of the workflow documentation
  • Explain Measure accurately, Act rapidly and Partner with patients (MAP) program
  • Discuss MAP program use in primary care
  • Describe the MAP implementation process
  • Examine the value of MAP program
  • Discuss the importance of using a data monitoring and assessment tool for hypertension management and control
  • Describe the mechanism and process for the implementation of a data feedback system
  • List components of the data feedback system to be evaluated
  • Explain the HEARTS program
  • Discuss use of HEARTS program in primary care
  • Describe the HEARTS implementation process
  • Examine the value of the HEARTS program in primary care
