Description
Hypertension is a long-recognized health burden in the US, and particularly the southeastern region of the US. Recent treatment guidelines have been developed to categorize blood pressure levels and treatment. This program addresses the implementation of the guidelines in primary care settings with a special focus for clinics serving underserved patient populations. This program focuses on the documentation, assessment, and implementation of hypertension control programs. Accreditation dates: 12/1/2023-11/30/2026
Objectives

At the completion of this activity, participants will be able to:

  • Describe the assessment of prediabetes in the primary care setting
  • Identify the documentation of prediabetes in the health record
  • Explain the treatment and management plan for prediabetes
  • Recognize the impact of undiagnosed hypertension
  • List factors associated with undiagnosed hypertension
  • Identify resources and practices to address undiagnosed hypertension in the primary care setting
  • Describe the assessment of Type 2 diabetes in the primary care setting
  • Identify the documentation of performance measures in the health record
  • State the treatment and management plan for diabetes
  • Examine use of the Category B Performance Measure in the primary care setting
  • Describe documentation of pharmacy involvement in the health record
  • Discuss the value of performance measures
  • Explain incorporation of the pharmacy and pharmacist in the assessment of hypertension in the primary care setting
  • Describe documentation of pharmacy involvement in the health record
  • State the treatment plan for hypertension with the pharmacist
  • Recognize the importance of using a data monitoring and assessment tool for hypertension management and control
  • Explain the mechanism and process for the implementation of a treatment monitoring system
  • Identify components of the system to be evaluated for hypertension treatment
