Description
In this presentation, participants will learn about the SC CHW Prioritization Index, understand how to use it, discuss its implications for CHW program design, and apply the data to identify opportunities for improving health outcomes in targeted areas.  
Objectives

At the completion of this activity, participants will be able to:

  • Explain what the SC CHW Prioritization Index is.
  • Provide a brief overview of how to use the index.
  • Utilize data from the index to identify areas of opportunity and how CHW programs can improve health outcomes in these areas.
